Most will require proof of vaccination for Tetanus-Diphtheria Td, Measles, Mumps, Rubella and Varicella (Chicken Pox)
VACCINATIONS
If you have no proof of vaccinations or blood tests to prove immunity, you will need to get vaccinations as determined by the Civil Surgeon. Our office can help you with locations for vaccinations that are appropriate for your age. You will then proceed to a location of your choice and receive vaccinations and bring evidence of vaccination back to the office. The cost of vaccinations is not included in the price of the exam and is payable directly to the vaccination provider.

Returning to the Office

As a final step, you will need to return to the office in approximately 7-10 days. This time period will allow for test results to come back and vaccinations to be taken. During this visit, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS and provide you a copy for your personal records. Please note that USCIS will not accept the form if the envelope has been opened or altered.
